Overall Summary:

Our proposed 04064 program focuses all of its energy in this one area: targeted outreach to deliver mobile HIV Counseling, rapid Testing, and client Referral (CTR) services. Our formative evaluation led us to narrowly focus on this area of intense need in specific southeastern New York State communities.

The four components above—combined with CDC support for this project and the intangible drive, know-how, endurance, and rigorous discipline of [Client ’s] management and field staff—will lead directly to a measurable increase in the proportion of HIV-positive Long Island residents who are aware of their HIV+ status. This critical awareness, based on CDC evidence-based learnings, will almost certainly lead to reduced high-risk behavior and a decreased number of residents at high-risk for HIV infection. By virtue of [Client's*] extensive referral relationships, this project will also provide a mobile portal to a broad array of comprehensive services for the individuals encountered in the proposed outreach program.

The CDC has proven that testing programs in non-clinical venues are more likely to reach persons at increased risk for HIV. [Client's*] proposed mobile CTR intervention seeks to operationalize this learning by introducing rapid testing to high-risk community residents on their ‘turf.’
